Forms for Voter Registration and advice

Registers of electors for Rural Representative Elections are compiled annually.

If you have not registered as an elector and wish to have your name entered in this year's register(s), please submit your application on or before 16 July of the same year.
If registered elector of Indigenous Inhabitant Representative Election wants to report on change of his/her principal particulars (i.e. name), he/she should submit his/her application on or before 16 June; for request for change of other particulars (including principal residential address, correspondence address or the Indigenous Village / Composite Indigenous Village to be registered), the application should be submitted on or before 16 July.
If registered elector of Resident Representative Election / Kaifong Representative Election wants to report on change of his/her principal particulars (including name or principal residential address), he /she should submit his/her application on or before 16 June. Please note that a registered elector of Resident Representative Election / Kaifong Representative Election applying for change of principal residential address (regardless of whether the principal residential address is still in the Existing Village / Market Town as registered) must attach with the application form the relevant required proof of principal residential address. For details, please refer to the Guidance Notes attached to the application form.
If an elector is required to confirm he still resides at his registered address as contained in the register of electors following the receipt of a letter from Home Affairs Department / the Electoral Registration Officer, the elector may be required to provide original or photocopy of proof of the registered address according to the information as mentioned in the letter.

In case the elector has no address proof, the Registration and Electoral Office will also accept the address proof of another person who lives with the elector. In such case, a declaration made by the elector is required to certify that such other person is living with the elector at the same address. In addition, the elector also needs to certify that the attached address proof is a complete authentic copy or a complete true copy of the original. The elector can also provide a statutory declaration made before a Commissioner for Oaths/a practising solicitor/a Justice of the Peace to substantiate the claim of residing at the registered address.

The elector may download a sample of the relevant declaration or statutory declaration as follows.
