Terms of Reference

The terms of reference are as follows –

  1. To steer the overall direction and implementation of District Services and Community Care Teams (Care Teams);
  2. to set criteria for assessing organisations and service proposals;
  3. to consider and approve the list of organisations passing the first stage of selection based on the recommendations of District Assessment and Coordinating Panels on Care Teams (District Assessment Panels);
  4. to consider and approve, in the second stage of selection, the formation of Care Teams on the basis of the service proposals from organisations passing the first stage of selection recommended by District Assessment Panels; and
  5. to monitor the overall performance of Care Teams and to decide on other matters relating to Care Teams at the territory-wide level.
