You may choose to use the free service available at our Home Affairs Enquiry Centres (Enquiry Centres) to administer declarations or oaths/ affirmations required of by non-governmental bodies (referred to as “declarations or oaths/ affirmations for private use”).
If a declaration or oath/ affirmation is required to be made upon the request of government departments (referred to as “official declarations or oaths/ affirmations”), you need to go to the relevant department for making the declaration or oath/ affirmation. Please click here to browse the types of official declarations or oaths/ affirmations administered by various government departments. Enquiry Centres will not administer these types of declarations or oaths/ affirmations.
To make a declaration or oath/ affirmation for private use, you may choose to approach any of our Enquiry Centres in 18 districts in person. You need to bring along with your documents to prove your identity (e.g. Identity Card). You are also required to complete a declaration or affidavit/ affirmation form as specified by the Home Affairs Departmentnote and read out the declaration or oath/ affirmation in front of our Commissioner for Oaths. If other documents are necessary, you may bring along these documents and attach them to the designated declaration or affidavit/ affirmation form as annexes. If the document you want to declare or swear/ affirm is duplicated from the original, you only need to bring along the duplicated document.
The table below shows the addresses, telephone numbers and the declaration or oath/affirmation service timeslots of the offices providing service on administration of oaths or declarations/ affirmations. Please note that the service timeslots may be subject to change without further notice, and you are strongly advised to consult the relevant Office in advance to confirm. Generally, you are required to arrive 15 minutes before the timeslot for preparatory work (e.g. completion of the form and stamping of the relevant documents).

Note: There are a few cases which the HAD’s specified form will not be used, e.g. the declarations relating to payment of accrued benefits as required by Mandatory Provident Fund Schemes Authority. Please see the declaration forms through the hyperlink.

If you have arrived or submitted documents late, you will be assigned to the next timeslot. Please note that 6:45 pm is the last timeslot for making declarations or oaths / affirmations for private use at the Kwun Tong Home Affairs Enquiry Centre.

Free Legal Advice Scheme
If you wish to seek free legal advice from the Scheme, you have to attend any of the referral agencies designated by the Duty Lawyer Service, including Home Affairs Enquiry Centres (Enquiry Centres), in person to make an appointment to meet volunteer lawyer at a Legal Advice Centre of your choice. At the time of making the appointment, our staff will conduct a preliminary interview with you to take down the detail background of your case which will be sent to the Duty Lawyer Service for vetting and processing. No appointment can be arranged for applicants who refuse to disclose details of their cases at the time of making appointment. Normally, you can meet the volunteer lawyer 6 to 8 weeks after the appointment interview.
Since the booking time slots for making appointments to meet volunteer lawyers vary among different Enquiry Centres, please call our Telephone Enquiry Centre at 2835 2500 during office hours (Monday to Friday from 9:00am - 7:00pm, except public holidays) for the booking time.
